Abstract Objective A cost-effective analysis of the laparoscopic total extraperitoneal (TEP) repair and trans- abdominal preperitoneal (TAPP) versus open inguinal hernia is still not well addressed. The aim this study to compare clinical outcomes TEP TAPP for unilateral hernia. Methods This a cost-effectiveness within randomized prospective conducted at Sanchinarro University Hospital. Cases primary, reducible unilaterl were included using simple randomization program. outcome parameters surgical postoperative costs, quality adjusted life years (QALY), incremental cost per QALY gained or effectiveness ratio. Results Between January 2020 2023, 60 patients enrolled in (30 them underwent 30 TAPP). procedure had less early pain (p=0.035). operative time was higher group with respecto (p =0.05). overall compared but without statically significance (883.93s vs 780 p>0,05). mean QALYs 1 year (0.7094) than that associated (0.541) (p=0,038).
